Remote & In-Person Services
Flexible Support to Meet Families Where They Are
At AI Community Initiative Inc., we understand that access to support should never be limited by location, transportation, or mobility challenges. That’s why we offer both remote and in-person case management and care coordination services to ensure individuals and families receive the help they need—when and how they need it.
🏠 In-Home Services
Our team is available to meet with clients face-to-face at community centers, homes, schools, or healthcare settings to provide:
- Comprehensive needs assessments
- Personalized care planning sessions
- Accompaniment to appointments or community resource visits
- On-site advocacy with providers or agencies
- In-person workshops and educational events
- On-site to assist with benefit application.
We maintain strict safety and confidentiality standards during all in-person visits.
💻 Remote Services (Virtual or Phone-Based)
We also provide a full range of virtual services for individuals and families who prefer or require remote support, including:
- Virtual intake assessments and case planning via secure video platforms
- Telephonic care coordination and wellness check-ins (Monthly and Quarterly Until discharged)
- Assist with text and email support for appointment reminders and referrals
- Remote mental health screenings and resource navigation
- Coordinate access to online support groups, webinars, and virtual workshops.
Our remote services are designed to be accessible, confidential, and convenient—especially for clients with limited transportation, childcare concerns, or health risks.
Representative Payee Services
AI Community Initiative Inc. is committed to financial stewardship for vulnerable individuals who need help managing their benefits.
As an authorized Representative Payee, we assist clients who receive Social Security (SSI/SSDI) or other government benefits and are unable to manage their own funds.
🔹 Our Role Includes:
- Budgeting funds to ensure needs like housing, food, clothing, and medical care are met
- Issuing personal spending allowances
- Ensuring timely bill payments and proper record-keeping
- Advocating for financial stability while maintaining the client’s dignity and independence
- We work closely with social workers, legal guardians, and service providers to support our clients' overall well-being.
